The Flatwoods Monster in Fallout 76 is disturbing due to its eerie, otherworldly design and unnatural presence, hearing tales of their horrors including claims of glowing eyes in the darkness makes them seem like a disturbing presence well before you actually see one .. Like an alien chupacabra. It straddles the line between alien and machine, which are easily the two scariest of the sci-fi genre, ET meets Wall-E, minus all the cute. With the in-game lore building its presence, its appearance (especially in dark forested areas) is shocking. Please dont abduct me, I cant handle another probing.

The Beast of Grafton is another creature in Fallout 76 that builds a LOT of its presence from in-game lore, including the Beast of Grafton holotapes as part of the Tales from West Virginia Hills series in game, a Twilight Zone style parody that turns out to be all too real. As youll see on this list, building lore and urban legend around a creature will inevitably create a disturbing story, and the Grafton monster lives up to this with grotesque size, distorted humanoid shape, and unsettling aura. Towering and lumbering through the misty West Virginian landscape, it emits guttural roars that echo ominously, signaling its presence before its seen. Terrifying.

Alright Im sorry, Ill stop talking about Fallout 76 soon, its just unfortunately filled with so many gross friends. The Wendigo Colossus blends the raw power of a Super Mutant Behemoth with immense health and resistances. Earle Williams, has over 30,000 HP and devastating melee attacks, which is a damn rough combo. He also passively reduces incoming damage by 80%, turning him into a massive bullet sponge. If you tried to clean your dishes with the sponge, itd break your f***ing hand.
Swan

Im gonna be so honest, this entry is 100% in here because of me, so if it gets negative comments My bad. I remember everything going really smoothly, I was wandering through the wasteland, enjoying my time, and then was foolish enough to step into the Swan Pond to see what the Swan thing in the middle was. Out of nowhere, this massive creature rises up and goes straight for me. It has no quest attachment, it has no story, its literally just there as a disturbing jumpscare, and goddamn does it work!
Frank Horrigan
Fallout 2 (1998)
Fallout 1 and 2 dont get their dues due to the mastery of the 3D series, but Frank Horrigan from the second game might be one of the most tragic characters in the series. Frank Horrigan was once an ordinary man serving as a loyal bodyguard to the president, until he was forcibly transformed into a Super Mutant, forcibly being the important part of this story. Now a walking tank, hes equipped with maxed-out stats, a deadly unique Plasma Gun, this guy can tear through your HP in seconds. Facing Frank alone is a death sentence, take advantage of your companions, youll need them.
Imposter Sheepsquatch

Back to Fallout 76! The Imposter Sheepsquatch, a heavily modified Assaultron, is REALLY upsetting to look at. Regardless of the look, the whole fight turns into a weirdly aggressive battlefield, forcing an odd amount of intensity. After using a keycard to summon it by hacking the terminal at the Pylon Ambush Site, waves of hostile robots flood the area. Youll need to fend them off while activating nearby pylons, which are key to making the Sheepsquatch vulnerable. This dude can take a beating. And look at its head! What the hell is even that?
Mothman

This is by FAR the worst part of the whole series for me, which might tell you why its here. I have legitimately diagnosed Mottephobia, a fear of moths, now look at this goddamn thing! I can barely edit this video. Having the presence of a giant moth, with glowing eyes, watching you in the darkness is as disturbing as it gets, but they also crawl across the ground, and tuck their wings in to block their face, theyre just the worst. Lets move on.
The Museum of Witchcraft Deathclaw

The Deathclaw is one of Fallouts most iconic and terrifying enemies. In the early pixelated games they were scary, but in 3D, they became towering, lightning-fast juggernauts capable of killing even high-level players in seconds. But theres one particular Deathclaw, in one particular location that is EXTRA disturbing. In the Museum of Witchcraft, there is one encounter that subverts expectation, and scares the living crap out of anyone who experiences it. As you explore the creaking, dimly lit museum, strange sounds echo through the halls, scraping claws, guttural growls, and distant crashes, creating unbearable tension. Clues hint at something horrifying stalking the building, and when the Deathclaw finally bursts into view, it's a violent payoff. Deathclaws dont usually stalk, so what the HELL is going on here?
The Ultracite Titan

Boasting a base HP of 30,000 and over 70% resistance to all forms of damage, the Ultracite Titan is a true ammo sponge, and unlike Earle Williams, theres no workaround, the special ultracite ammo made specifically for creatures like this does absolutely NOTHING. Bringing down this massive creature requires a coordinated team and top-tier weaponry just to make a dent. On top of that, its goddamn disgusting, its like a mole thats also morbidly obese.
