Top 10 Live Choreography Fails

Welcome to MsMojo, and today we’re counting down our picks for the most cringe-worthy moments when live choreographed performances went hilariously wrong.

#10: Katy Perry’s Dancer Falls off Stage

“37th Brit Awards” (2017)

Katy Perry’s performance of “Chained to the Rhythm” at the 2017 BRIT Awards already had eyeballs on it thanks to its politically-tinged visuals, skeleton puppets, and house-shaped backup dancers. But one of those dancers inadvertently stole the show. As Perry strutted around the stage, the unfortunate house-clad dancer misjudged their position and tumbled right off the edge of the stage. The dancer disappeared into the pit below while the performance continued without missing a beat. The moment quickly went viral, with viewers comparing it to yet another infamous Katy Perry choreography fail. Thankfully, the dancer was okay, but they certainly brought the house down that night – literally.

#9: Meghan Trainor Takes a Tumble

“The Tonight Show Starring Jimmy Fallon” (2016)

May 12th 2016 was the day Meghan Trainor learned the hard way that ankle-breaking heels and slick TV studio floors don't mix. After nailing her performance of "Me Too" on “The Tonight Show,” Trainor turned to grab the microphone stand. That's when her towering stilettos betrayed her, sending the Grammy winner tumbling to the floor. True to his gentleman nature, Fallon immediately joined her on the ground in solidarity, turning the embarrassing spill into a charming, shared moment of laughter. Trainor handled it like a pro, but those heels definitely weren't all about that bass.

#8: Beyoncé's Flying Car Malfunction

Cowboy Carter Tour (2025)

Even Queen Bey can't control technical difficulties. During Beyoncé’s massive Cowboy Carter Tour stop in Houston in June 2025, her performance of “16 Carriages” was meant to be a jaw-dropping spectacle. The singer sat atop a suspended red convertible prop that lifted and flew over the audience as part of the choreography. Partway through the number, however, the car suddenly tilted and began to lean alarmingly. The music cut, and Beyoncé calmly called “Stop, stop, stop…” into her mic as she clutched a harness cable and waited to be lowered. No one was hurt, and after her safe descent she thanked her fans and forged ahead with the show.

#7: Michelle Williams Trips & Falls

“106 & Park” (2004)

Long before viral TikTok dance fails, Destiny’s Child had a memorable choreography slip on BET’s “106 & Park.” During a live performance of their hit “Soldier,” member Michelle Williams took a misstep at the beginning of the number and fell onstage mid-entrance. In classic pro-performer fashion, she jumped right back up and carried on with the choreography as though nothing had happened. However, clips of her fall have lived on through fan recordings and YouTube uploads for years. Although Williams has now embraced the tumble, she has acknowledged that the experience and the ridicule afterwards had a traumatic effect on her.

#6: Normani’s Unexpected Descent

“Good Morning America” (2017)

Fifth Harmony brought serious energy to “Good Morning America” when they debuted their single “Down” in 2017. Known for her athletic choreography and smooth transitions, Normani was in the middle of a high-impact dance sequence when she slipped on stage and hit the deck. Rather than freeze, she instantly regained her footing and jumped back into formation like a seasoned pro. Her groupmates continued the performance without drawing attention to the mishap, but eagle-eyed fans caught the moment. Still, the way Normani recovered from the tumble actually highlighted her dance skills rather than detracting from it. That's how you turn a potential embarrassment into a demonstration of star quality.

#5: Lil Nas X's Wardrobe Malfunction

“Saturday Night Live” (2021)

In May 2021, Lil Nas X brought his provocative “MONTERO (Call Me By Your Name)” song to “Saturday Night Live” with a performance that quite literally split at the seams. Mid-routine, while executing a sensual pole dance sequence, the artist dropped into a squat that proved too much for his leather pants. The feeling of the fabric tearing was immediately followed by a look of panic crossing his face. Rather than stopping, Nas strategically covered his crotch with his hand while continuing the choreography one-handed. In post-show interviews, he revealed the dancers were supposed to touch him afterwards, but he had to wave them off to prevent further exposure.

#4: Jennifer Lopez’s “Louboutins” Slip

“37th Annual American Music Awards” (2009)

The 2009 “American Music Awards” witnessed J.Lo's rare moment of human fallibility. While performing her single "Louboutins," Lopez executed an ambitious choreography sequence that involved climbing a staircase of male dancers arranged in a human pyramid. The dramatic moment came when she attempted to jump from the top, but instead of sticking the landing, she slipped and tumbled. In true Lopez fashion, she bounced back up within a split second and continued her routine as if nothing had happened. The audience barely even had time to gasp. This spectacular recovery showcased why J.Lo has stayed relevant for decades – when she falls, she falls with style, and gets right back up again.

#3: Katy Perry's Left Shark

“Super Bowl XLIX Pepsi Halftime Show” (2015)

No list of choreography fails is complete without this one. During the 2015 Super Bowl XLIX halftime show, Katy Perry’s troupe included two dancers in shark costumes performing alongside her “Teenage Dream” set. While the shark on the right kept up with the choreography, the left shark didn’t. Its moves were unmistakably off-beat, loose, and frankly improvised, creating a hilariously mismatched contrast with the tightly drilled routine. The internet went nuts, memes exploded, and “Left Shark” immediately entered the pop culture lexicon. What started out as a choreography disaster turned into a cultural phenomenon, even spawning trademark battles over the character. Left Shark proved sometimes the most memorable performances come from going way, way off-script.

#2: Madonna's Cape Catastrophe

“35th Brit Awards” (2015)

The 2015 BRIT Awards saw the Queen of Pop suffer one of live television's most dramatic falls. During her performance of "Living for Love," Madonna made a grand entrance wrapped in a flowing Armani cape. The choreography called for a dancer to dramatically pull the cape away, revealing her costume underneath. Unfortunately, the cape's ties were fastened too tightly and refused to release. Instead of the cape gracefully flying off, Madonna was yanked backward, tumbling down three steep stairs. The horrified gasps from the audience were audible as the then 56-year-old hit the floor hard. Being the seasoned pro that she is, Madonna barely paused, picking herself up to finish the performance despite revealing she later cried out of humiliation.

#1: Janet Jackson's Wardrobe Malfunction

“Super Bowl XXXVIII AOL TopSpeed Halftime Show” (2004)

The wardrobe malfunction that launched a thousand FCC complaints occurred during Super Bowl XXXVIII's halftime show in 2004. As Janet Jackson and Justin Timberlake performed his hit "Rock Your Body," the choreographed finale called for Timberlake to pull away part of Jackson's costume. Instead of revealing a red lace garment underneath as planned, the move tore away more material than intended, exposing Jackson's breast to around 150 million viewers for approximately half a second. The incident, which Timberlake infamously dubbed a "wardrobe malfunction," sparked nationwide controversy. Jackson's career suffered disproportionate consequences while Timberlake's continued to soar, raising questions about double standards that persist to this day. Not only did this choreography fail go viral, it changed American television forever.
