Although it's meant to be a battle royale between seven people and their servants, plenty of uninvolved people are killed off. Ryunnosuke and Caster kill several innocent people, many of them children, and Kiritsugu Emiya's backstory is rife with people getting killed all over.

While there's no end of the world, just about every death that can be onscreen is onscreen. The first episode alone has many murders, Shinichi kills plenty of complete parasites, and the body count racks up significantly as the series progresses.
